Thank you for choosing Metropolitan Urology as your urologic specialist. In this section, you will find information which will briefly explain some office procedures and policies.
Please remember–your health is important to us. If you are unable to make your appointment, please call us as soon as possible so that we may reschedule you or offer your time to another patient. Introduction

Urology is a specialty of medicine concerned with disorders of the urinary tract in both males and females and with the genital organs in the male. Our practice specializes in both adults and children with these disorders.

New Patient Information Forms

Before you arrive for your appointment, select and click on the appropriate New Patient Information Form from the list below, print out the form, complete the form, and bring with you to your appointment.

Centralized Scheduling
Appointments are scheduled in advance at each office location. To enable us to make the most convenient appointment for you please call as early as possible.
You are urged to keep your appointment or give 24 hours notice of cancellation. This will provide the opportunity for another patient in need of care to be seen.

What To Bring
Please bring with you all medications you are currently taking and a list of any drug allergies you may have. Also, please have available any records from previous physicians regarding your current condition, as well as any laboratory work and/or X-rays. If X-rays have been performed within the past 12 months, please stop by the institution where they were taken and retrieve the films as well as a copy of the radiologist’s report. This will eliminate any unnecessary delays or repetition of tests.

Phone Calls, Prescriptions, and Emergencies
The office staff will be able to answer all non-medical and many medical questions. Many telephone calls and questions can be avoided by bringing a member of the family with you for the office visit. Requests which cannot be dealt with by the office personnel will be communicated to one of the physicians.

Prescriptions are best refilled during office hours when records are available for review.
A physician is always available to handle emergencies. However, routine questions are best handled during office hours when your records are available to your physician.

Hospitalization
Our physicians are on staff at most Southern Indiana and Louisville area hospitals. The choice of hospital will be discussed with you by your physician. If possible please call our office before going to a hospital emergency room. IN THE EVENT OF ANY LIFE THREATENING EMERGENCY CALL 911.

Insurance- Please Read Before Your Appointment

  1. If our office participates with your insurance company we will file your primary and secondary insurances. If we do not participate with your insurance company you will be responsible for your entire bill.
  2. You will receive a statement only if a patient balance is due.
  3. If your insurance requires a referral, it is your responsibility to make sure you have the referral at the time of service, otherwise this may lead to excessive delays or rescheduling of your appointment. If you fail to obtain the necessary referral, you will be responsible for your bill, in full.
  4. All co-pays are to be paid at the time of service. If you are unable to pay at that time, we will need to reschedule your appointment.
  5. We will fill out your first disability form at no cost; for additional forms there will be a $10.00 fee.
  6. Some elective surgeries may require partial or full prepayment of the patient due portion before they can be scheduled.
  7. You will need to bring all insurance cards with you on the first visit. Payment at the time of service will be requested if these are unavailable.
  8. There are certain diagnoses that are not covered by your insurance. Please refer to your insurance booklet or contact your insurance company regarding your benefits.
  9. Billing calls are taken between the hours of 9:00 A.M. and 5:00 P.M. Monday through Friday.

Locations

Our main office is located in Jeffersonville, Indiana and is open Monday through Friday from 9:00 a.m. to 5:00 p.m. This is the office where all billing and insurance inquiries can be answered. Our other offices are open only when doctors are scheduled to see patients. For your convenience all calls are forwarded to our Jeffersonville location.

Privacy Practices

Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. It is given to all new patients and a copy is available upon request.
